Ms. Suzi <br />Ericksen informed me that a Succession of Operators would need to be completed to change the <br />permittee from Robert V. Mayne to Mayne Industries, Inc. The proper form to complete this succession <br />of operators is enclosed. This completed form, plus an application fee of $125.00 must be submitted to <br />the DMG Denver Office to initiate the process. <br />Also enclosed is the financial warranty form you requested to place a cash bond in the State Treasury. <br />If you have any questions regarding completion or submittal of these forms, please contact Ms. Suzi <br />Ericksen in our Denver office.
