In your <br />initial submission of May 4, 1995 which was amended with your explanation of what a <br />Technical Revision criteria should be, and a check for $875.00 to cover the fee for a DMO <br />Technical Revision application on May 19, 1995, seven items were listed for consideration by <br />the Division. <br />The following items have been approved through the Technical Revision No. 13 review <br />process. <br />Item No. 1: Preparation of soil liner for areas to be completed under Phase I. <br />Item No. 3: New source of underdrain rock material for Phase II. <br />Item No. 5: Winterization plan for Phase II pad area. <br />Item No. 6: Surface water control drainage plan for Phase II construction seazon. <br />The following Items included in the Technical Revision are denied until the operator provides <br />the following information and receives written approval by the Division: <br />Item No. 2: Use of pad 2 material to construct the Phaze II toe berm, the Highway 67 <br />realignment embankment, and will be used az a selected structural fill for the <br />downstream face of Phase II toe berm. If the material meets all the engineering <br />requirements, the Division will have no problem in approving the request. <br />Please provide the required engineering tests that the material meets all the <br />requirements. As to its suitability in terms of geochemistry, please refer to <br />
