On behalf of our client, Connell Resources, Inc., we aze submitting a request for a Technical
Revision to the above mentioned permit. We are also submitting the attached Contingency Plan
as required by the Mined Land Reclamation Boazd by permit conditions #5, 6 and 7. Attached is
an application fee check for $150.00.
The requested Technical Revision to the approved permit consists of changing the locations of
the Phases of the mining and adding two Phases. To help develop the required Contingency Plan
the phases of the approved mining operation were shifted and changed to accommodate the need
to allow continued groundwater flows through the permit azea and downgradient to the Swift
Ponds for a portion of the life of the mining operation. The attached Exhibit C, Mining Plan
map, shows the new proposed phases. The new phasing does not change the proposed total
disturbance at any one time.
Tlie Contingency Plan (condition #5) was developed during a series of discussions between
ourselves, Connell and Mr. Swifls' representative, Mr. John Andrew. The plan that is attached
with this letter has been provided to Mr. Andrew and Mr. Swift. Several revisions have been
coordinated between all the parties.
The Contingency Plan consists of monitoring information as it relates to the phases of the mining
operation, with critical points for determining impacts to adjacent properties. The attached
Contingency Plan details [he required monitoring of the two sites. Also attached is an aerial
photo showing the location of the four additional piezometers on the Swift property (cardition
#6). These piezometers have been surveyed in and elevations established for the purposes of the
moni [ori ng.
