On August 16, 2006, the Division of Reclamation, Mining and Safety received a Financial Warranty Certification of
Deposit and Horizon Banks Certificate of Deposit No. 224103, in the amount of $7,670.00, for O & A Stone, Permit No.
M-1994-121. The Division is returning these documents for the following:
Financial Warranty Certification of Deposit:
The Division requires [he Operator name listed throughout this document match the Operator name listed on the
Succession of Operator application. The Operator name on the Succession of Operator application is O & A Stone.
The Division requires the operation name (i.e. O & A Stone) to be consistent throughout this document.
The Division requires the amount of the CD be listed alphabetically on page 2.
The Division requires the completion of date information pertaining to the CD on page 2.
The Division requires the Operator name listed on the Assignment of Certificate of Deposit match the Operator
name listed on the Succession of Operator application. The Operator name listed in the Succession of Operator
application is O & A Stone.
Certificate of Deposit:
The Division requires the Operator name match the Operator name listed on the Succession of Operator
application. The Operator name listed in the Succession of Operator application is O & A Stone.
The Division requires the our PDPA No. be listed on the Certificate of Deposit and Signature Cazd.
The Division requires proof of a lock or hold from the bank issuing the CD.
