During this period, Ponds T (Outfall 002A), B (Outfall 003A), D (Ouffall OO5A), E (Outfall 006A), and F (Outtall 007A) <br />discharged to Foidel Creek. No permit excursions were noted for this period. Outfalls 003A, 005A, and 006A flowed each <br />week of the period. Outfall 002A flowed only during the third week of the period, Ouffall 006A flowed all but the third week of <br />the period, and Outfall 007A flowed the last two weeks of the period. No permit exceedances were noted. <br />Quarterly Whole Effluent Toxicity (WET) tests were conducted for Outfalls 005W and 007W in February and March 2005, <br />respectively. Both tests indicated that the effluent were not acutely toxic to Daphnia magna survival. <br />Outfalls 004A (Pond C), and 008A (Pond G) did not discharge; and therefore had no chemical tests during the period. Outfall <br />020A/020W has not yet been constructed, therefore sampling of this site has not occurred. Note that ponds A (Outfall <br />001A/001W), and H (Outiall 009A) have been deactivated following bond release, and have been removed from the newly <br />amended permit, effective January 1, 2003. In order to reduce the amount of paper work, the following certification will be <br />substituted for the actual DMR forms to cover the outfalls, which did not discharge. <br />CERTIFICATION <br />1 certify under penalty o/law that I have personally examined and am familiar with the in/ormation submitted herein; and <br />based on my inquiry o/ those individuals responsible /or obtaining the information, I believe the submitted information is <br />true, accurate, and complete. 1 am aware that there are signi/icon[ penalties forsubmitting false information, including the <br />possibility of fine and imprisonment. See 18 U.S.C. § 1001 and 33 U.S.C. § 1319. (Penalties under these statutes may <br />includa fines up to $10,000 and/or maximum imprisonment of between 6 months and 5 years.) <br />Brian A.
