LOng <br /> Division Dveclor <br />Re: H & H Stone Quarry # 3, File No. M-2000-154, Receipt of 112 Comtruction Materials Reclamation Permit <br />Applieation Package <br />Dear Mr. Holley: <br />On December 8, 2000, the Division of Minerals aad Geology received your 112 Construction Materials Reclamation Permit <br />Application package for the H & H Stone Qtarry q 3, File No. M-2000.154,. Preliminary review of the information received <br />de[errmined that the fotlowing items must be received before the Division can consider your application as being submitted and <br />technical review can begin: <br />APPLICATION FORM: Page 1, Item 3: The exact number of acres must be filled in. Page 2, Item 9: Only one <br />quarter section and one quarter/quarter section should be indicated. The general description needs completing. Page <br />2, Item 10 and I I: Only one area should be checked. Page 7, Signature Page: The Division requires that the raised <br />corporate seal be aftv:ed to this page. The Division requires that the operator name and the notarized nartte be the <br />same (i.e. H & H Stone Co., Inc.). <br />E)QiIBIT N -Source of Leeal Ridrt to Enter (Rule 6.4.141: You must provide the source of your legal right to <br />enter and initiate a nvnutg operation on the affected land. <br />E7aHBIT R - Pnoof of Filine with Cormty Clerk and Recorder (Role 6.4.18): You must submit an affidavit or <br />receipt indicating the date on which the application was placed with the local County Clerk and Recorder. <br />Your application will not be considered submitted until the information listed above is received and found sufficient [n begin <br />our review. A decision date will be established 90 days fiortt the date of receipt of atl of the requested infornation. <br />Additionally, if you have already published notice you will need to republish notice, after the Division comsiders the application <br />submitted. This motive must be published once a week for four (4) consecutive weeks, starting within ten (10) days of the date <br />your application is considered submitted. We will notify you when you should initiate republicatiom of your notice. The final <br />date for receiving cormnem(s is the 20th day aRer the fourth publication or the next regular business day. <br />This letter shall not be construed to mean that there are o0 other technical deficiencies in your application.
