The report aiso indicates any changes in operations from chat <br />was stated in the original Notice of Intent sent to the Division. The <br />' Operator may use this farm, modify this form, or uce a different format <br />altogether . <br />' 1- Attach a map that shows: <br />a) Which proposed holes have been drilled to date, <br />b) Any actual or proposed relocations of holes, <br />' c) The extent of all significant road construction completed to <br />date (exclude minor blade work), and <br />' d) Current location of all drill rigs and test equipment. <br />' 2. OescriDe what and where reclamation activities have occurred 1n the <br />past 180 days. Mention sealing and plugging, regrading, retopsolling and <br />reseeding activities.
