At that time you will need to submit the enclosed form along with a copy of the bond release from the <br />Division of Minerals and Geology. Also, you must certify that all discharges to state waters have ceased. Upon <br />receipt of the completed termination form, the District Engineer (D.E.) for the above-referenced county will be <br />notified and will inspect your facility. Once the D,E. has approved the inactivatioq the permit will rio to public <br />notice fora 30-day comment period. Regulations require that monitoring reports be submitted until the <br />permit is formally inactivated. If appropriate, such reports should simply state "no discharge". <br />Please be awaze that Section 61.15 {e) {ii) of the permit regulations state: <br />"Once the Division proceeds to terminate a permit at the permittee's request, the prorated fee shall <br />apply to the period of time the permit has been in effect, including but not exceeding ninety (90) <br />days from the date the permit termination request is received by the Division." <br />Please contact me at (303) 692-3556 with any questions.
