The assistance of Bill Beaz was very helpful and greatly appreciated. I have enclosed a copy <br />of the inspection report for your records. <br />INDUSTRIAL WASTEWATER and STORMWATER SYSTEM <br />The Bear No. 3 Mine is an inactive facility. Wastewater is generated from stormwater control and mine <br />seepage. Wastewater dischazges from these facilities aze regulated under CDPS Permit #CO-0044377. <br />The Division administratively extended this permit on January 31, 2001. There aze two permitted <br />outfalls. <br />• Outfa11001 follows a settling pond for groundwater drainage from old mine portals. This outfall <br />flows on a continuous basis. It was dischazging coffee-colored water during our inspection, and it was <br />observed dischazging the same type of water on August 3, 2000 and again in 2002. Facility monitoring <br />records indicate a continuous dischazge as well. Outfa11001 is located at 38°54.05 north, 107°36.88 <br />west. <br />• Outfa11004 follows a settling pond for surface runoff from the mine. This outfall has not <br />dischazged in the past 2 yeazs. It is located at 38°54.26 north, 107°37.06 west. <br />Violations <br />During the itspection, the following violations were noted and need to be brought to your attention: <br />J.
