III IIIIIIIIIIIIIIII <br />999 <br />BLUE MOUNTAIN ENERGY, INC. <br />Spill Prevention, Control and Countermeasure Plan <br />HISTORY OF SPILLS <br />There have been no reportable spills at the Deserado Mine in the last 10 years. <br />The only reportable spill occurring on the site was in 1986 when approximately 1500 gallons <br />of hydrochloric acid was released due to a faulty valve. Appropriate actions were taken to <br />minimize contamination and to clean up the site following the spill. There were no notable <br />long-term impacts. Secondary containment was enlarged to allow full containment of similaz <br />occurrences in the future. <br />RESPONSIBLE FACILITY MANAGEMENT <br />The Mine Manager, Safety Manager, appropriate Supervisor and Envirorunental Engineer <br />will be notified as soon as a leak or spill of any hazardous material is found. The source and <br />cause of the spill or leakage will be defined and appropriate action taken to minimize <br />additional releases and environmental damage. The period of the event will be defined as <br />to date, time and duration. Steps will be taken to identify the cause of the leak or spill, the <br />nature of the product, where the leak or spill originated, where the product could migrate, <br />health hazards: etc. A Spill Report Form (Attachment 1) is to be filled out by the responsible <br />parties. Action will then be taken to prevent a recurrence of the event. <br />Action to be taken will be based on the type, quantity and nature of product involved. The <br />goal would be to contain the event and prevent damage to adjacent areas on or off site. <br />These actions may include, but are not limited to: <br />1. Dikes or berms <br />2. Absorbent booms <br />3. Diversion ditches leading to containment <br />4. Available neutralizing agents <br />5. Absorbent agents <br />AGENCIES TO CONTACT IN CASE OF REPORTABLE SPILL <br />STATE WATER QUALITY CONTROL DIVISION <br />The State Water Quality Control Division will be notified within 24 hours after a leak or spill <br />is found. A written report will be mailed within 5 days. The source and cause of the leakage <br />or spill will be defined and, if possible, actions will be taken to prevent a reoccurrence.
