The Division agrees that it would <br />be appropriate and desirable to seal this monitoring well in accordance with the <br />requirements of Rule 4.07.3. and permit section 2.05.4(2)(8). <br />Please be reminded that within 60 days of sealing of the well Landmark needs to submit <br />Proof of Compliance in accordance with Rule 4.07.3(3): <br />Proof of compliance. Within 60 days after abandoning a drill hole, specific drilling <br />program approved by the Division, or when requested by the Division, the operator <br />shall submit a report detailing the following information: <br />(a) Location of the drill hole as spotted accurately on a topographic map; <br />(b) Depth of drill hole; <br />(c) Surface elevation of drill hole; <br />(d) Intervals where water was encountered during drilling activities; <br />(e) Diameter of drill hole; <br />(f) 'Cype of cement or other approval sealant used; <br />III IIIIIIIIIIIII III ~~ <br />999 <br />STATE OF COLORADO <br />(g) Amount of cement and volume and slurry mixture or other sealant used in <br />plugging drill hole; <br />
