Before the Division can <br />consider terminating the above-referenced permit, a release of bond from Division of M1tinerals and Geology is required. A[ <br />that time you will need to submit the enclosed form along with a copy of the band release Crom the Division of Minerals and <br />Geology. Also, at that time, you (must cemfy that all dischazges to stale waters bout ceased. Upon receipt of the completed <br />termination form, the District Engineer (D.E.) for the above-referenced county will be notified and will inspect your facility. . <br />Regulations require that monitoring reports be submitted until the permit is formally terminated. if appropriate, such reports <br />should simply state "no dischazge". <br />Please be awaze that Section 6.16L0(5) (B) of the permit regularioas stale: <br />"Once the Division proceeds to tcrmina[e a permit a[ the permittee's request, the prorated fee shall apply to the period <br />of time the permit has been in effect, including but no[ exceeding ninety (90) days from the date thte permit termination <br />request is received by the Division." <br />Please contact me at (303) 692-3599 with any questions concerning fees.
