Cottony <br />Acting Division Director <br />The subject report was submitted on January 14, 2003, and received at the Division on January <br />17, 2003. This review is conducted to determine compliance with Rule 4.05.9(17). <br />Rule 4.05.9(17) All impoundments shall be examined at least quarterly, by a qualified person <br />designated by the operators, for appearance of structural wealmess2 and other hazardous <br />conditions3- The reports shall be retained at or near the mine site, and a copy of the report shall be <br />submitted to the Division on a quarterly basis°, or other approved submittal frequency. <br />I The inspection was accomplished by a registered engineer. <br />2 The pond was inspected for structural weakness. <br />' The pond inspected for other hazardous conditions. <br />° The inspection was submitted to the Division on a quarterly basis.
