Your <br />assistance was very beneficial in helping me evaluate the current wnditions at the site, as well as understanding <br />your proposal regazding consolidation of outfalls 002, 002 and 006. <br />Enclosed is your copy of the inspection form completed for this inspection. If you have any questions with the <br />form, please contact me. <br />There were no deficiencies noted that require immediate attention. It appears that the permit accurately depicts <br />the facility configuration, with the exception of outfall 005 that was never constructed. You may want to request <br />deletion of that outfall if it will not be constructed. This could be part of your outfall consolidation request. <br />If the facility conditions change as a result of mining changes, please notify the Division propmtly so that a <br />determination may be made regazding whether the permit needs to be modified to reflect the changing conditions. <br />Please contact me at (303)+692-3605 if you have questions. <br />Sincerel , <br />Jon C.
