Long <br />/ / ~ Division Director <br />Re: O'Dwyer, Ptile Na M-2002-017, Receipt of 110 Construction Materials Reclamation Permit Application <br />Package <br />Deaz Mr. Blake: <br />On February 22, 2002, the Division of Minerals and Geology received your the 110 Construction Materials Reclamation <br />Permit Applipticn package for the O'Dwyer, File No. M-2002-017. Preliminary review of [he information received <br />determined that the following items must be received before the Division can consider your application as being submitted <br />and technical review can begin: <br />EXHIBIT A -Leval Description (Rule 6.3.1(1)1: The legal description must identify the affected land, specify <br />affected azeas and be adequate to field locate the property. Description shall be by township, range, and section, to at <br />least the nearest quarter-quarter section. A metes and bounds survey description is also acceptable. Also, the street <br />address or lot number(s) may be used. This information is available from the County Assessor's office or U.S. <br />Geological Survey (USGS) maps. <br />EXHIBIT J -Proof of Mailing Notices of Permit Apnlication (Rule 6.3.10): Include proof of mailing of the <br />"Notice of Permit Application" sent to the Board of Supervisors of the High Plains Soil Conservation District, in the <br />application, by submitting the date-stamped receipt for Certified Mailing or adate-stamped copy of the notice form <br />with signature aclmowledging persoaal service and receipt by the board Also, refer to Rule 1.6.2(lxa). <br />Your application will not be considered filed until the information listed above is received and found sufficient to begin our <br />review. A decision date will be established 30 days from the date of receipt of all of the requested information. Additionally, <br />if you have already published notice you will need to republish notice, after the Division considers the application submitted <br />Republication must occur once within ten (10) days of the submittal date. We will notify you when you should initiate <br />republication of your notice. The final date for receiving comments is ten (10) days after the date of publication or the next <br />regular business day. <br />You have sixty (60) days from the above date to submit all necessary documents that the Office needs for an application to be <br />considered filed L`, at the end of the sixty day period, the application has not been determined Lo be filed with the Office, the <br />Office shall detry the application and tP*minate the application file. <br />This letter shall not be construed to mean that there are no other technical deficiencies in your application.
