Outfalls <br />003A and 005A flowed the first, fourth and fifth weeks of the period. Outfall 005A flowed only during the fourth week of the <br />period, allowing only one sample to be collected. No permit excursions were noted for this period. Note that Ponds T (Ouffall <br />002), C (ouffall 004) and G (Ouffall 008) did not flow during this quarter, therefore no samples were collected from these sites <br />for quarterly conductivity testing. <br />Quartedy Whole Effluent Toxicity (WET) testing was conducted for Outfall OOSW in June 2004. The effluent was not acutely <br />toxic to Daphnia magna survival. Note that Pond F (Outfall 007W) did not flow during this quarter, therefore no sample could <br />be collected for quarterly W ET testing. <br />Outfalls 002A (Pond T), 004A (Pond C), 007A (Pond F), and 008A (Pond G) did not discharge in June; and therefore had no <br />chemical tests during the period. Ouffall 020A/020W has not yet been constructed, therefore sampling of this site has not <br />occurred. Note that ponds A (Ouffall 001A/OO1W), and H (Ouffall 009A) have been deactivated following bond release, and <br />have been removed from the newly amended permit, effective January 1, 2003. In order to reduce the amount of paper work, <br />the following certification will be substituted for the actual DMR forms to cover the ouffalls, which did not discharge. <br />CERTIFICATION <br />1 certify under penalty o/law that I have personally examined and am familiar with the in/ormation submitted herein; and <br />based on my inquiry o/ those individuals responsible /or obtaining the information, I believe the submitted information is <br />true, accurate, and complete. I am aware that there are significant penalties for submitting /alse information, including the <br />possibility of fine and imprisonment. See 18 U.S.C. § 1001 and 33 U.S.C. § 1319.
