During this period, only ponds B (Ouffall 003), D (Ouffall 005), and E (Outfall 006) discharged to Foidel Creek. Note <br />that these Outfalls flowed during the first, third and fourth weeks of the period, with no flow from any of the three during the <br />second week of the period. No permit excursions were noted for this period. Further note that Outfalls 001 (Pond A), and 009 <br />(Pond H) have been deactivated, and have been removed from the newly amended permit, effective January 1, 2003. <br />Outfalls 002, 004, 007, and 008 did not discharge; and therefore had no chemical tests during this period. Outfall 020 has not <br />been constructed yet; therefore there is no discharge to enable chemical testing to date. Points 001, 009, 010, 016 and 017 <br />have been eliminated from this permit. In order to reduce the amount of paper work, the following certifica8on will be <br />substituted for the actual DMR forms to cover the outfalls, which did not discharge. <br />CERTIFICATION <br />1 certify under penalty o/law that I have personally examined and am familiar with the information submitted herein; and <br />based on my inquiry of those individuals responsible for obtaining the information, I believe the submitted information is <br />true, accurate, and complete. 1 am aware that there are significant penalties for submitting false information, including the <br />possibility of fine and imprisonment. See 18 U.S.C. § 1001 and 33 U.S.C. § 1319.
