A slope stability analysis for structures within 200 feet of the affected area was performed
by Lyman Henn Inc. The original report, dated March 2004, was performed to show
stability of the Windsor water pipeline. An addendum, dated 10 May, 2004, also includes
the Eaton ditch, Everitt barn and Poudre Trail. The slope stability analysis assumes
reclamation slopes of 3H:1 V, and the applicant does commit to reclaiming the water
storage reservoirs to no steeper than 3H:1V.
2. Proof of notification to property owners, easement holders, and other affected parties on or
within 200 feet of the permit boundary.
3. Affidavit of Receipt from Weld County for submittal of additional completeness documents.
The newspaper public notice run ended yesterday (May 11, 2004) and I will forward you the
"affidavit of publication" when received. We understand that the public comment period will end
on June 1, 2004, and that the decision date is set for July 13, 2004.
Please call me if you have any questions or require additional information about this application.
Thank you.
Sincerely,
~~~
Maureen Jacoby
Project Manager
attachments: Lyman Henn Slope Stability Analysis, dated March 2004
Addendum to Slope Stability Analysis, dated 10 May 2004
Proof of Notifications (15 certified mail receipts)
Affidavit of Receipt from Weld County, dated April 14, 2004
cc: Mr. Martin Lind, Hilltop Aquatic Investments, LLC
Mr. John Vazquez, P.E., JCB Engineering, LLC
Weld County Clerk to Board
