A report on the /acility's overall compliance with the SWMP. (Include here a summary of any measures <br />taken to comply with your Stormwater Management Plan (SW NiP), to fully implement it, changes or improvements <br />made in any of your Bes! Management Practices (BMP's), problems encountered, spills, other potential violations, etc.) <br />To the best of my knowledge, for the period of October 1, 2003 through September 30 2004 the Twentvmile <br />Coat Companv (TCC) was in substantial compliance with the terms and conditions of CDPS <br />Permit No. CO-0027154 and the terms and conditions of all permits and regulations necessary to conform <br />with the Stormwater Management Plan (SWMP) developed for this facility. <br />The Best Management Practices (BMPs), inspections, maintenance activities, and training requirements <br />specified in the SWMP for TCC were adhered to throughout the year. No changes to the BMPs: no problems, <br />spills, or other potential violations were noted or encountered. <br />Were changes made to your SWMP? ~No Yes -Describe changes on a separate sheet. <br />B. A summary of each comprehensive Stormwater facility inspection made, including date, findings, and action taken. <br />(The permit requires at least two comprehensive facility inspections per year -see Part I page 1 of 14 of the permit <br />Include here a summary of [hose inspections, plus any other comprehensive inspections made. It is not <br />necessary to summarize day-to-day inspections, unless significant problems were noted.) <br />First Inspection -Date 6/15&16/04 Findings, and action taken <br />A comprehensive inspection of drainage control structures. diversion structures, vegetated slopes, silt fences, berms, <br />vegetative filters, rock filters, and storage areas for raw materials, chemical products, fuels, hazardous substances. <br />and waste products was completed on the TCC facility. Maintenance items noted included minor repair/clearing of functional <br />culverts. Storage areas were found in good order. <br />Second Inspection -Date 9/7&8/04 Findings and action taken: <br />A comprehensive inspection of drainage control structures, diversion structures. vegetated scopes. berms, <br />vegetative filters rock filters and storage areas for raw materials. chemical products fuels hazardous substances, <br />and waste products was completed on the TCC facility. The roads throughout the mine are in good condition, <br />rock check dams are functional. Storage areas were found in good order.
