This permit application was approved on March <br />16, 2004, A formal approval letter will be mailed from our Denver office to Moffat Limestone <br />Company, Inc. under separate cover. <br />Please review the enclosed figures as soon as possible and contact our office if any calculation <br />errors are noted. A financial warranty in the total bond amount shown on the Cost Summary Form <br />and a performance warranty will need to be submitted to, and accepted by DMG, prior to issuance <br />of a permit for this operation. A performance warranty form and letter of credit financial warranty <br />form are enclosed for this purpose. The letter of credit form was enclosed because this is the type <br />of financial warranty submitted by Moffat Limestone Company, Inc. for the 110c permit at this <br />same location.
