Chronology We submitted the application on November 19, 1995. I had heard nothing by December 12th, and called Bruce Humphries to see what was going on. He informed me that a request for more information was in the mail. Lavern Lemay notified me (letter dated December 11, 1995) that the "Responsibilities as a Permittee" list in the application form had not been initialed as required. I hand- delivered an initialed list to Jim Stevens on December 19, 1995. Mr. Stevens informed us (letter dated December 21, 1995) that the comment and review periods began on December 19, 1995, and that the decision date for our application is January 19, 1996. You then began processing the application. 1) Proof of Publication Given the above chronology, we first published in the Aspen Times on November 25, 1995. Subsequent correspondence from DMG indicated we should republish. We republished on December 23, 1995. Copies of both publications are attached, as well as a Proof of Publication for the December 23rd publication.
