Dear Mr. Goode:
On May 25, ?000, the Division of Minerals and Geology received your the 110 Construction Materials
Reclamation Permit Application package for theB&DTrucking,FileNo.M-2000-072. Preliminary review of
the information received determined that the following items must be received before the Division can consider
your application as being submitted and technical review can begin:
APPLICATION FORM: Page 1, Item 1, Applicant/operatoror company name: Needs clarification.
Page 1, Item 3, Permitted acreage: Please enter the exact acreage. Page 6, Signature Page: The
Division requires [hat the Applicant/Operatorbe notarized as the same names as listed on Page 1, Item
]. The Division requires the submittal of one complete signed and notarized ivoryoriginal,onecopyof
the completed ivory application form, and two copies of Exhibits A-J, Exhibit L and Addendum 1.
EXHIBIT B -Site Description (Rule 63.2(a)): You must address Item (a) to the extent necessary to
demonstrate compliance with the applicable performance standard requirements of Rule 3. At a
minimum, you must include a description of the vegetation and soil characteristics in the area of the
proposed operation. The local office of the Natural Resources Conservation Service (MRCS) may
provide you with this informational well as recommendationsfor Exhibit D -Reclamation Plan;
EXHIBIT B -Site Description (Rule /i.3.2(b)): You must address ]tem (b) to the extent necessary to
demonstrate compliance with the applicable performance standard requirements of Rule 3. At a
minimum, you must identify any permanent man-made structures within two hundred (200) feet of the
affected area and the owner of each structure. Permanent man-made structures include houses, barns,
fences, above or below ground utilities, irrigation ditches, maintained or pubic roads, bridges, wells,
pipelines, water storage impoundments, railroad tracks, cemeteries, and communication antennas, etc.
Each structure should be located on Exhibit E -Map;
EXHIBIT B -Site Description (Rule 6.3.2(c)): You must address Item (c) [o the extent necessary to
demonstrate compliance with the applicable performance standard requirements of Rule 3. At a
minimum, you must give a description of the water resources in the area of [he proposed operation.
Identify any streams, springs, lakes, stock water ponds, ditches, reservoirs, and aquifers which would
