Preliminary review of <br />the information received determined that the following items must be received before the Division can consider <br />your application as being submitted and technical review can begin: <br />APPLICATION FORM: Page 1, Item 1, Applicant/operatoror company name: Needs clarification. <br />Page 1, Item 3, Permitted acreage: Please enter the exact acreage. Page 6, Signature Page: The <br />Division requires [hat the Applicant/Operatorbe notarized as the same names as listed on Page 1, Item <br />]. The Division requires the submittal of one complete signed and notarized ivoryoriginal,onecopyof <br />the completed ivory application form, and two copies of Exhibits A-J, Exhibit L and Addendum 1. <br />EXHIBIT B -Site Description (Rule 63.2(a)): You must address Item (a) to the extent necessary to <br />demonstrate compliance with the applicable performance standard requirements of Rule 3. At a <br />minimum, you must include a description of the vegetation and soil characteristics in the area of the <br />proposed operation. The local office of the Natural Resources Conservation Service (MRCS) may <br />provide you with this informational well as recommendationsfor Exhibit D -Reclamation Plan; <br />EXHIBIT B -Site Description (Rule /i.3.2(b)): You must address ]tem (b) to the extent necessary to <br />demonstrate compliance with the applicable performance standard requirements of Rule 3. At a <br />minimum, you must identify any permanent man-made structures within two hundred (200) feet of the <br />affected area and the owner of each structure. Permanent man-made structures include houses, barns, <br />fences, above or below ground utilities, irrigation ditches, maintained or pubic roads, bridges, wells, <br />pipelines, water storage impoundments, railroad tracks, cemeteries, and communication antennas, etc. <br />Each structure should be located on Exhibit E -Map; <br />EXHIBIT B -Site Description (Rule 6.3.2(c)): You must address Item (c) [o the extent necessary to <br />demonstrate compliance with the applicable performance standard requirements of Rule 3. At a <br />minimum, you must give a description of the water resources in the area of [he proposed operation. <br />Identify any streams, springs, lakes, stock water ponds, ditches, reservoirs, and aquifers which would <br />receive drainage directly from the affected area.
