Boz 2379 <br />Colorado Spring, CO 80901 <br />Re: Receipt of ll2 Coostrudion Materials Reclamation Permit Application, Table Mountain Quarry, <br />File No. M-99004 <br />Deaz Mr. Pazaday: <br />On February 1, 1999, the Division of Minerals and Geology received your 112 Construction Materials Reclamation <br />Permtt application for the Table Mounrrin Quarsy, which is located in Fremom County. All comment and review periods <br />began on February 1, 1999. The decision date for your application is scheduled for May 3, 1999. <br />Within ten (10) days after submittal, you must publish notice in a newspaper of general circulation in the locality of the <br />proposed mining operation ortce a week for four (4) consecutive weeks. Within that same [en (10) day period, you must <br />mail or personally serve a copy of the same information provided in the public notice to the newspaper to the owners of <br />record of surface tights and all owrers of record of surface lands that are wishing 200 feet of the botmdary of the affected <br />lam. U.nrnniafrly after the first publication, mail a copy of the notice to this office. Proof of notice am mailings, such <br />as Certified Mail - Retum Receipt Requested, must be submitted to the Division of Minerals and Geology prior to the <br />decision date. Additionally, any changes or additions to the application on fde in our office must also be reflected in the <br />public review copy which has been placed with the Fremont County Clerk and Recorder. <br />The Division is reviewing your application to determine whether it is adequate to meet the requirements of the Act.
