Our initial review indicates that two items are missing. First, <br />the application form is unsatisfactory. I am sendiny you the appropriate <br />form required by the Division. Secondly, a newspaper advertisement is <br />required. Enclosed with this letter is a suggested format that is to be <br />submitted with the permit application. For the purpose of finding an <br />application complete, the remainder of your submittal is fine. The <br />Division will not begin a review until the application form is submitted. <br />As we discussed in our meeting .September 4, 1981, it is permissible for <br />Blue Flame Coal mine to continue mining provided that no additional surface <br />disturbances are incurred and no more than 250 tons of coal per year are <br />produced. To insure that Blue Flame is complying with the latter require- <br />ment I am enclosing a monthly production form. This is to be sent in at <br />the end of each month to this Division.
