Long <br />Divrsion Dueaor <br />Re: Cugnini Gravel Pit, Permit No. M-1986-061, Request for Extension of Time to Perform Corrective <br />Actions. <br />Dear Mr. Jacobs, <br />The Division is in receipt of the written request for the second extension of the corrective action date, regarding <br />problems at the above-named site. The report of the April l9, 2001, inspection lists two problems, bath of which <br />originally had May 25, 2001, corrective action dates. Though Mr. Patrick Cugnini is the permittee, it is with your <br />office that we are communicating in regard to these problems and their corrections. <br />The problems are still outstanding, and you have requested extensions to be able [o correct them. Your initial <br />request fora 30-day extension was approved, and the date was reset to Monday, June 25, 2001. This second <br />request is for an additional 30 days. This request is also approved, and the corrective action date is now extended to <br />July 25, 2001.
