Long <br />Di~i lion Director <br />Based on staff complaints concerning operator relations with DMG <br />staff, and a meeting with CC&V management, Golder QA/qC staff, and <br />DMG management, it was agreed that the DMG staff will include the <br />following in the inspection close-out meeting and inspection <br />reports and following procedure: <br />1. Any instances where the inspector determines that the <br />inspector has been unduly delayed or hindered in conducting the <br />inspection. <br />2. Any instance where the inspector determines that the Golder <br />construction QA/QC representatives have displayed actions that <br />defend the methods or actions of the operator or changes to <br />approved permit construction specifications. Golder does have the <br />right to discuss how tests are conducted, but not the number of <br />tests or retesting that may be requested by DMG staff. The <br />operator does have the right to question the number of tests and <br />retesting. <br />In addition, no verbal approvals of material specification changes <br />shall be allowed from this point forward. Changes to material <br />specifications must be submitted in writing by the operator. Those <br />changes must be submitted to Mr. Keffelew. Approval or denial of <br />any changes to material specifications must be directed through the <br />lead specialist ( Mr. Keffelew) and signed off by the Minerals <br />Program Supervisor. This will ensure that the operator does not <br />get mixed information or contradicting approvals. <br />cc: Mike Long <br />CC&V <br />Golder Associates, (CCSV) <br />
