Dear Ms. Gaboriau:
The Division received a paper copy of an e-mail pertaining to your request for an extension of a
corrective action completion date for a corrective action outlined in the Division's May 6, 2004
inspection report. Specifically, the Division instructed Aggregate Industries to submit a plan with
specific construction details and an abatement timetable to correct a potential problem with the
existing spill control structure that houses a large fuel_tank by July 25, 2004 (see May 6, 2004
inspection report). Aggregate Industries requested a 90-day extension of the corrective action
due date via an e-mail transmission. You were told that a-mail requests aze not acceptable and
that you needed to submit a paper copy request for the extension of the corrective action due
date. The Division issued an approval of your request because it was assumed that you would
send a paper copy of the request within a few days after the a-mail request was sent. However,
you did not submit a paper copy within a few days after you sent the a-mail request, so the
Division approval was withdrawn.
You need to send a paper copy request for the extension of the corrective action due date.
According to Mr. H. Bruce Humphries, Minerals Supervisor, the request needs to be on
Aggregate Industries letterhead and a signature is required.
