To the best of my knowledge at this time, there were two construction related incidents that contributed to the loss of the fresh water. 1. On August 24, 1998 there was a break in the PVC piping above the Hazel 5~1 A adit. This was quickly discovered because of a loss of water pressure at the 5 hose being used for compaction. The situation was corrected within 3¢ to 45. minutes by shutting down the pumping system and running new pipe. It is . o estimated that less ihan a 000 gallons of fresh water was released at a point above the Hazel A and drained across the road and part way down Cash Gulch. l~ ,y~e~° 2. The second incident began late on August 24, 1998 and was discovered and corrected (by tightly closing the valve) early on August 25, 1998. It is estimated that approximately 3000 gallons of fresh water was lost. As a corrective action, we have now instituted a process of shutting down and visually checking the water systems before leaving the site.
