Walther <br />Exetmive Director <br />Michael8 Long <br />Division Director <br />Re: Cugnini Gravel Pit, Permit No. M-1986-061, Request for Extension of Time to Perform Corrective <br />Actions. <br />Dear Mr. Jacobs, <br />The Division is itt receipt of the written request for the fourth exteDSion of the corrective action date, regarding <br />problems at the above-named site. This fourth request is for an additional month, in order to make the needed <br />changes and corrections. Even though your request was received after the existing correction date, this office is <br />approving your request, and the corrective action date is now extended to September 25, 2001. <br />As I stated in my last letter to vou, dated July 23, 2001, if you require an extension~ast the third one, I would <br />require an interim problem correction status report. I am hereby remindinc vou that your interim report is now due. <br />This has been the last approved extension for correctine these Droblems. Please ensure that the required corrective <br />actions are completed and that vou contact me with written evidence of such corrections prior to the new deadline <br />date. <br />Please be reminded [hat when problems are noted for non-compliance with approved permit conditions, and simple <br />corrective actions are required to be performed, timely performance is expected. -I-hese have not been large, <br />complex problems t0 correct. If the upcoming corzective actioD date is not met, it would be prudent for all of us to <br />block out time on our calendars for presenting our cases at a Mined Land Reclamation Board hearing. <br />I look fonvard to your submittal documenting the satisfactory completion of the corrective actions.
