Preliminary review of the information received determined that the following <br />additional information must be received before the Division can consider your application complete and <br />technical review can begin. Thus, your application is incomplete for the purposes of filing as of November <br />12, 2024. <br /> <br />Per Rule 4.17.1(2) the Operator shall include names, addresses, and phone numbers of all landowners of <br />record for the affected land. The names and addresses are provided, but not all phone numbers are provided. <br />Please resubmit page 2 of the request form to include phone numbers for all the record landowners. <br /> <br />You have thirty (30) days from November 12, 2024 to submit all necessary documents that the Division <br />needs for the application to be considered filed.
