Colorado Inactive Mine Reclamation Program
CLOSEOUT REPORT
ACTIVE MINE BOND FORFEITURE PROJECTS
PERMIT INFORMATION
Permit Number Operator Name Mine Name
M-1994-091 Lucio Vasquez Lucio Vasquez Quarry
Type of Mine: 110c- Construction Limited Impact Commodity- Sandstone
PRE-RECLAMATION SITE CONDITIONS
The Lucio Vasquez Quarry is located approximately 2 miles northeast of Lyons in Boulder County. The 110c
permit was issued to Lucio Vasquez in 1994 for a permit area of 9.9 acres. In 2011, the Mined Land
Reclamation Board issued a violation to Lucio Vasquez for mining outside the approved permit boundary (0.9
acres) and was required to post a bond in the amount of $22,600.00 to reclaim the site. The Operator failed to
correct the violations. As a result, the permit was revoked and the bond money in the amount of $3,787.00 was
forfeited.
BIDDING/CONSTRUCTION INFORMATION
Type of Bid Documented Quote -. - • •er
(circle one) Invitation for Bid_ Direct Bill
PO Number: Contractor Name & Address: Construction Bond Amount (after 5%
Dates: administrative fee removed):
RMC Consultants, Inc. $3,598.00
122295 W48th Avenue Start: 11/16/2015
Wheat Ridge, CO 80033 •
Seed $1,085.00
• Labor $2,513.00
End: 11/16/2015 Project Cost: $3,598.00
Construction Work Description
The permit required the mine slopes be reclaimed to 2H:1V and 4H:1V using existing waste rock piles and topsoil.
Because of the insufficient amount of bond to reclaim the site, the only feasible option was to reseed the 10.9- acre
disturbed area. A reclamation seed mixture, containing grass, forb and shrub species common to the area, was
purchased from Arkansas Valley Seed Company. RMC hand broadcast the mixture over all disturbed areas
including waste piles and flattened slopes.
Remaining Reclamation Requirements and Estimated Cost: Remaining reclamation work includes grading
down the waste rock piles, removal of debris and trash and control of noxious weeds at the site.
74VA-/ (Ait) 4.1"7
Project Manager Senior Prriz fr an g r
I i Si Tots' la sli5
Date Date
1
