(OSMI) received Adequacy Review -3 for Technical Revision 12 to Permit <br /> M2012-032 for the Revenue-Virginius Mine(Mine). The Adequacy Review raised 2 major points, each <br /> of which are addressed in italics under headings that repeat the DRMS point of clarification. <br /> l. Your response, including the supporting regulatory information for the proposed abandonment of <br /> GW-4 is noted and accepted. However, no discussion of the plugging and abandonment method <br /> of GW-4 was included.'Please provide details of the chosen method of plugging and <br /> abandonment of GW-4, in accordance with Rule 5.4, including a tentative schedule of when that <br /> activity will take place. <br /> Response: The area around the well will be dug down I foot and the well casing will be cut there. The <br /> well will be filled with bentonite and the top 3 feet of the well will be plugged with concrete. The ]-foot <br /> hole will be filled with 3-inch rock to match groundcover in the area around the well. The well will be <br /> abandoned in late September or early October once the Division approves TR12. OSMI surface crews <br /> will perform the work. <br /> 2. The proposal of visual leak detection of Pond 3 is also noted and accepted,please provide a <br /> commitment to monthly visual monitoring for leak detection of the outslope of Pond. Reporting <br /> documentation will be provided with the quarterly Groundwater Monitoring Report. <br /> Response: The mine commits to monthly visual monitoring for leak detection along the outslope of Pond <br /> #3. A form has been created in Fulcrum (a fellable form application) and monitoring will begin <br /> immediately.
