The ROM was placed in a manner consistent with the
requirements outlined in Technical Revision (TR) 103, VLF2 Ore Stacking Guidelines.
However, the practice of placing larger material (< 3") against drain cover fill (DCF) is not
typical at CC&V.
The ROM placed on the 9900' elevation was dumped from a height of—30' above DCF, well
within the requirements of TR103. The largest rock within the ROM load was —4' in diameter.
There is no suspected damage to the DCF or liner, however CC&V has scheduled a 3rd party
contractor, Newfields, to arrive April 9, 2019 to inspect the area and provide findings and
recommendations.
Figure 1 provides an overview of the area ROM placement schedule. No ROM was scheduled to
be placed in the area of concern. Despite proper routing through the Ore Control group and ore
tracking software, a haul truck operator inadvertently routed the ROM to the area.
