Everist, Incorporated, respectfully requests a Technical Revision for the Firestone Gravel Resource mine <br /> permit, DRMS Permit M-1996-052. The revision is being submitted to address a Corrective Action noted in the <br /> DRMS Inspection Report for the Firestone site, dated 10/23/2018. This technical revision is for the purposes of <br /> updating and clarifying the current approved mine and reclamation plans, and to have the plans better reflect the <br /> existing and proposed activities. We have included the Request for Technical Revision (TR) Cover Sheet(form) <br /> and Check#1897 for$216.00 to cover the Technical Revision fee. <br /> For this Technical Revision, we are updating the following: <br /> -Mining Plan and Mining Timetable <br /> - Reclamation Plan and Reclamation Timetable <br /> - Reclamation Bond analysis and table <br /> -Acreage Corrections <br /> Mining Plan <br /> The mining methods have not changed from what is approved in the current permit. We have updated Exhibit <br /> Cl -Mining Plan Map to update the areas of disturbance-including current, new and proposed settling ponds; <br /> an area to be stripped (of topsoil and overburden); a drying pad (for settling pond fines); and oil &gas facility <br /> changes (some wells have been abandoned and removed). We have also revised Exhibit Cl to show the <br /> updated permit boundary approved with the October-2018 acreage release. <br /> We have also revised the Mining Timetable to reflect the changes listed above, changes from the October-2018 <br /> acreage release, and two small acreage corrections due to minor errors by DRMS and by L.G. Everist. (See <br /> details below in Acreage Correction section.) <br /> Reclamation Plan <br /> The reclamation methods have not changed from what is approved in the current permit. We have updated <br /> Exhibit Fl -Reclamation Plan Map to show revised lake areas, which are increasing due to oil &gas wells and <br /> facilities being abandoned and removed. The parts of Area 2 which are current or proposed settling ponds will be <br /> lake area with final reclamation —they will just be reclaimed to a shallower depth as indicated on the map. Exhibit <br /> F has also been revised to show the updated permit boundary approved with the October-2018 acreage release. <br /> We have also revised the Reclamation Timetable to reflect the changes listed above, changes from the October- <br /> 2018 acreage release, and two small acreage corrections due to minor errors by DRMS and by L.G.
