Actions taken to respond to and correct the emergency situation or condition <br /> • Around 9:25 pm on September 29, 2018, the barren solution pumps at the ADR2 barren solution <br /> tank temporarily shut down. Consequently, the tank overtopped, spilling approximately 4,000 <br /> gallons of barren solution on the North side of ADR2. The spill occurred over the triple-lined <br /> Pregnant Solution Storage Area(PSSA). All solution was contained and remained on liner. <br /> • Upon discovery,the barren solution pumps were restarted and the incoming flow was stopped. <br /> • The material has been over-excavated and clean fill brought in to eliminate the potential for <br /> tracking to unlined areas. <br /> • This spill was due to a physical valve malfunction and process personal are working to <br /> investigate the issue and prevent repeat events. <br /> b. Any known or adverse impacts to human health,property or the environment <br /> • No impacts to human health or the environment were observed or are expected to occur because <br /> all solution remained on liner and quickly infiltrated into the PSSA. <br /> 1 <br />
