Actions taken to respond to and correct the emergency situation or condition <br /> • On August 26, at approximately 3:00, a leak was observed in a slurry tailings line between the <br /> process ore thickener to the filter presses. Approximately 100 gallons of slurry tailings spilled <br /> outside of secondary containment at the HGM before the leak was identified and repaired (see <br /> photo 1). <br /> • Approximately 1 cubic yard of slurry and soil was over-excavated and placed at the Load Out Bin <br /> (LOB) tailings stockpile to be mixed with leach ore and placed on Valley Leach Facility 2 <br /> (VLF2). <br /> b. Any known or adverse impacts to human health,property or the environment <br /> • No impacts to human health or the environment were observed or are expected to occur due to the <br /> low quantity spilled,the benign chemical composition of the tails, and the swift clean up. <br /> 1 <br />
