Actions taken to respond to and correct the emergency situation or condition
• At approximately 1:30 am on July 21, 2018, process personnel were cleaning out the secondary
containment at the HGM Concentrate Thickener when a transfer pump hose moved, spilling
approximately 150 gallons of slurry outside of secondary containment. The portable pump was
immediately shut off and cleanup actions initiated.
• A skidsteer was used to recover the slurry and impacted soil. The soil was removed using an `over-
excavation' approach. Depth of recovery was approximately two to six inches.
• Approximately 20 cubic yards of slurry and soil were transported by the skidsteer to the HGM ore
stockpile. The slurry and over excavated soil were mixed with ore and will be fed back into the HGM.
Clean gravel was placed in the excavated area.
b. Any known or adverse impacts to human health, property or the environment
No impacts to human health or the environment were observed or are expected to occur due to the low
quantity spilled, the benign chemical composition of the concentrate, and the swift clean up.
1
