The following items are incomplete for <br /> the application to be processed: <br /> 1. Financial Warranty Check for deposit in State Treasury Third Party is required for the <br /> check of $6000.00 provided by Outlaw Gravel for Operator James Shelton, per Bank <br /> notation on the check. Form enclosed. <br /> 2. Structure Agreement was not provided for Simla Highway, aka CR 125 from El Paso <br /> County. See instructions enclosed for Structure Agreements and sample agreement. <br /> Please return the required documentation to the Division before July 29, 2018 to complete <br /> the application for approval. All completed permit documents are required to be submitted <br /> for consideration of your Request for Transfer of Mineral Permit and Succession of Operators <br /> Application. An incomplete application will be denied after the 60 day submittal request, if <br /> requested documents or information are not received.
