The application Question#8 has been filled in with the name of the owners of the surface of <br /> the affected lands and a revised page three has been resubmitted. <br /> 2. Application Question#11 has been changed with the updated coordinates to indicate the <br /> actual coordinates. It appears on the revised page as well. <br /> 3. A new page six was updated with Responsibility#10 left blank. <br /> 4. A new Exhibit A map has also been resubmitted that shows sufficient information for the <br /> affected land on the ground and labeled the access routes to be used with the mining <br /> operation. <br /> 5. A new letter was filed with the Clerk& Recorder to show proof that the deficiencies were <br /> taken care of.
