Actions taken to respond to and correct the emergency situation or condition <br />At approximately 10:00pm, Mill operators identified process water coming from the Processed Ore <br />Thickener Overflow Tank. Process water was flowing out from the top of the overflow tank, into <br />secondary containment. The secondary containment filled with process water from the tank overflow, <br />eventually overtopping the walls of the secondary containment. Approximately 4,000-5,000 gallons of <br />the neutral pH process water exited containment and onto the lined area adjacent to the containment <br />(photo 1). <br />• Mill operators noticed the pumps at the overflow tank were not operating properly at the time of the <br />spill. It was determined that the system pump program was updated earlier in the day, consequently the <br />overflow tank pump was not running in "auto". The operator switched the pump control to "auto", <br />immediately stopping process water from flowing out of secondary containment. <br />• The improvements made to the overflow pipe after the August 10"' spill prevented the processed water <br />from splashing out of containment between the containment and the HGM. <br />Two front end loaders were used to recover the saturated soil. The soil was removed using an `over - <br />excavation' approach (photo 2). Depth of recovery was approximately six to twelve inches. <br />4 <br />
