RE: Hayden Gulch Loadout (Permit No. C-92-081) — Permit Renewal (RN05) Application
Dear Mr. Musick:
Hayden Gulch Terminal, LLC, LLC (HGT) respectfully requests that the CDRMS proceed with the renewal
process for our existing approved Mining and Reclamation Permit (Permit C-92-081), which expires on May 22,
2018. Attached is the completed application form for "Renewal of a Valid, Existing Permit to Conduct Mining
in Colorado'. The permit should be renewed for an additional term of five years in the name of the current
permittee, as follows:

Attached, consistent with the applicable requirements for permit renewal of Rule 2.08.5(2)(b), are the following
supplemental materials:
Copy of proposed Public Notice, consistent with applicable requirements of Rule 2.07.3(2)
Copy of current liability insurance policy (renewed annually each year during the term of the active
mining permit)
