The following items <br /> remain incomplete for the application to be processed: <br /> 1. Structure Agreements were not provided for all applicable structures per the permit file, Exhibit S <br /> Permanent Man-Made Structures, compared to Structure Owners submitted with the Succession <br /> of Operator Application. Proof of mailing to all Structure Owners is required pending return of <br /> completed Structure Agreement. <br /> 2. Request for 60 day extension to provide Proof of Mailing to all applicable Structure Owners, and <br /> Completed Structure Agreements <br /> Please return the required 60 day Extension Request as soon as possible for approval and revised <br /> due date. <br /> All completed permit documents are required to be submitted for consideration of your Request for Transfer <br /> of Mineral Permit and Succession of Operators Application. An incomplete application will be denied after <br /> the 60 day submittal request, if requested documents or information are not received.
