Dear Mr. Russell:
The purpose of this letter is to respond to the Incompleteness Notice for the aforementioned Limited
Impact(110) Operation Reclamation Permit Application for consideration in Cheyenne County,
Colorado. Each section of the Incomplete Sections is listed below.
APPLICATION FORM
1. The Applicant indicated Jolly Pit#3 for Question 1.1 on Page 1 of the application. This is
likely an error as this question requests the Type of Organization filing of the permit. Please
revise this by identifying that the Applicant is Local Government and submit a new page 1 of
the application. Attached is a revised page 1 of the application identifying that the
Applicant is Local Government.
2. The Applicant initialed Responsibility#10 on page 5 of the application. The Division believes
this is an error because the Applicant/Operator is not a joint venture or partnership. Please
submit a new page 5 of the application with all but#10 initialed. Attached is a new page 5
of the application with all but#10 initialed since Applicant/Operator is not a joint
venture or partnership.
3. The certification page (Page 7 of the application) contains errors that will need to be corrected
before the application is considered complete for filing purposes. The Applicant/Operator is
identified as Rod Pelton's signature and is signed as Cheyenne County instead of the vice
versa. Additionally, the notary section identifies Rod Pelton as the Owner/Operator of
Cheyenne County, but his title is identified as Chairman. Please submit a new certification
page with the revised Applicant/Operator,being signed correctly, and having the correct title
of the signer identified in the notary section. Attached is a new page 7 of the application
