Following is a summary of enclosed items as requested. <br /> 1.6 Public Notice <br /> 1. As required by Rule 1.6.2(d) and 1.6.5(2),proof of publication by affidavit from <br /> the "REPORTER-HERALD", is enclosed. <br /> 2. As required by Rule 1.6.2(e),please find enclosed Certified Mail receipts from <br /> the U.S. Postal Service as proof of the notice to all owners of record of surface <br /> and mineral rights of the affected land and the owners of record of all land surface <br /> within 200 feet of the boundary of the affected land including all easement <br /> holders located on the affected land and within 200 feet of the boundary of the <br /> affected land. <br /> 3. Loveland Ready-Mix Concrete, Inc. will maintain the existing augmentation plan <br /> as decreed in Division 1 Water Court case no. 2009CWI05. Should the need arise <br /> for another permit; a formal permit application will be filed. Loveland Ready-Mix <br /> Concrete, Inc. will also follow requirements under CRS 24-80 part 13 if any <br /> possible cultural resources are uncovered. <br /> 6.4.5 Exhibit E—Reclamation Plan <br /> 4. The text"A copy of the approved USR amendment will be provided when <br /> available"has been added to page 11 of 39 of the permit application. Enclosed is <br /> the replacement sheet 11. <br /> 6.4.12 Exhibit L—Reclamation Costs <br /> 5. Loveland Ready-Mix Concrete, Inc. agrees with the Divisions bond estimate and <br /> adopts the estimate as part of the application. Pages 21 and 22 of 39 shall be <br /> replaced with the attached AMND 21 of 39 and AMND 22 of 39. <br />
