Financial Warranty Check for Deposit in State Treasury Third Party is required. New Financial <br /> Warranty is enclosed to complete and return to the Division. <br /> 4. Right of Entry from Landowner to Rock Pile,LLC not provided. <br /> 5. Structure Agreement from Structure Owner to Rock Pile,LLC not provided. <br /> Dustin Czapla,Environmental Protection Specialist will return to the office on October 24,2016 and will confirm <br /> any additional incomplete items. <br /> Please return the required documentation to the Division before December 18,2016 to complete the application for <br /> approval.
