The decision date for this Technical <br /> Revision is September 12, 2016. The Division sent the Operator an Adequacy Review Letter <br /> dated March 14, 2016. The following items identified in the previous letter have yet to be <br /> addressed: <br /> 1. The Applicant is proposing to leave the perimeter berms in place. In addition, the Operator is <br /> proposing to connect the berms along the north and west edges of the site. Please confirm the <br /> berms to be constructed will match the approximate dimensions (height, width, outslope) of <br /> the existing berms. <br /> 2. Please provide an estimate of how much material will be required to construct the berm and <br /> specify where the material will come from. <br /> 3. Please specify how the berms will be stabilized. Will the berms be seeded? If the berms will <br /> be seeded, specify the seed mix to be used, the seeding rate (PLS/acre), and seed application <br /> method. <br /> 4. The Reclamation Plan Map prepared by Coffey Engineering& Surveying does not conform <br /> to the required mapping criteria. The Reclamation Plan Map must be revised to contain the <br /> following information: <br /> a. Depict the entire permit area. <br /> b. Contain a labeled permit boundary. <br /> c. Contain an appropriate title (Reclamation Plan Map), <br /> d.
