Everist, Inc. <br /> Contact: Steven O'Brian - 303-423-7297 <br /> Section 6, Twp 2 North, Rng 67 West, 6th P.M., Weld County <br /> CONDITIONS FOR APPROVAL <br /> L.G. Everist, Inc. requested an amendment to the application for Carbon Valley Resource, Permit no. <br /> M2001-017. The purpose of this amendment is to add 56.4 acres to the permitted area, for a total of <br /> 124.4 acres. The final reclamation of the site will still be clay lined or slurry wall lined water storage <br /> reservoirs. The proposed operation will continue to consume ground water through evaporation, dust <br /> control, reclamation, water removed with the mined product, and dewatering. The Carbon Valley <br /> Resource is included in a Substitute Water Supply Plan, which covers depletions caused by gravel <br /> mining operations at the site through December 31, 2017. Also well permit no. 70326-F has been <br /> approved for this site. So long as the amended is operated in accordance with an approved SWSP or <br /> approved plan for augmentation, this office has no objection to the request. Any expansion <br /> associated with the mining operation (evaporation and operational uses) beyond the limitations in <br /> the SWSP or the well permit would require the Applicant to obtain a new well permit and amend the <br /> current SWSP. <br /> If stormwater runoff is intercepted by this mining operation and is not diverted or captured in <br /> priority, it must be released to the stream system within 72 hours, which may require a discharge <br /> permit from CDPHE-WQCD.
