Everist, Inc. respectfully requests an Acreage Release for fully completed reclamation work in the <br />Meadows Lake and Meadows West phases of the Fort Lupton Sand and Gravel Mine, M-1999-120. <br />Both phases/cells have been completely reclaimed for release as developed water storage as per <br />general DRMS reclamation standards and specific permit conditions detailed by the DRMS in the <br />Preliminary Review letter of AM -02 (dated 05/29/12) and agreed to by L.G. Everist, Inc. <br />1. SEO Certification and Approval of Liner <br />Meadows Lake - the slurry wall liner was approved by the SEO in August -2012. Meadows Lake was <br />called "Everist Reservoir No. 1" in the approval letter. A copy of the SEO's approval letter is <br />attached for your reference. (Meadows Lake has had various names throughout permitting, title <br />work, and survey work: Everist Reservoir No. 1, or D-(0)7. And prior to TRO7 in September -2015, <br />Meadows Lake was a combined phase named Meadows North and South.) <br />Meadows West - the slurry wall liner was approved by the SEO in January -2016. A copy of the <br />SEO's approval letter is attached for your reference. (Meadows West has had various names <br />throughout permitting, title work, and survey work: Everist Reservoir No. 2 and No. 4, or D-(0)8 and <br />D -10S.) <br />2. All bank sloping has been completed to the stated 3:1 slopes or flatter. <br />3. All areas that are above the high water line, and are not graveled access roadways, have been <br />topsoiled and seeded as per the seed mix described in AM -02. Effective weed control measures <br />are being implemented until the cell is released from the permit area. A copy of the seeding invoice <br />from Geisman Seeding Service (dated 4/5/16) is attached for your files. <br />4. Establishment of full revegetation is not required for release as per our permit conditions, because <br />the City of Aurora is taking over the property soon (June/July-2016) and will continue development <br />of the water storage reservoirs. A copy of the Special Warranty Deed between L.G.
