Preliminary review of the information received <br /> determined that the following items must be submitted before the Division can consider your <br /> application as being submitted and technical review can begin: <br /> 1. The Applicant only submitted Form 1 (confidential). In order to file the application for <br /> review, the Applicant must provide a copy of Form 2 (non-confidential). Pursuant to Rule <br /> 5.1.2(h)(ii), the Applicant must submit two separate forms. One form will contain all <br /> information, including both public and confidential information. This complete form will be <br /> used by the Office for review and will be held confidential. The second form will contain <br /> only the information the Applicant believes is public with the Applicant redacting all <br /> information to be held as confidential. Please note that confidentiality may only apply to <br /> information relating to the mineral deposit location, mineral deposit size or nature, and <br /> information believed by the Applicant to be proprietary or trade secret or that would cause <br /> substantial harm to the competitive position of the Applicant. <br /> 2. The Applicant did not provide initials under the Signature Requirement section of Page 9 on <br /> Form 1. Please read this section carefully and provide initials next to each of the signature <br /> requirements. In addition,please note that one of the signature requirements includes <br /> providing notice of the NOI application to the Board of County Commissioners in the County <br /> where the proposed activities will occur. A blank copy of Page 9 has been enclosed with this <br /> letter. <br /> 3. The Applicant has listed Puma City LLC as the name of the company in which the NOI will <br /> be issued.
