Preliminary review of the information received determined that the following <br />items must be received before the Division can consider your application as being submitted <br />and technical review can begin: <br />APPLICATION FORM: The applicant /operators names must match exactly as <br />registered with the Colorado Secretary of State. The business name is registered <br />as "White River City Investments LLC ". Please fill out page one (1) of the <br />application form with the correct registered business name. <br />APPLICATION FORM: Page eight (8) of the application form must be properly <br />signed and notarized with the applicants name (Don Valentine) rather that the <br />inspection contact (Don Silva). <br />• Business / Trade Name: A search of the Colorado Secretary of State database <br />revealed that the applicant trade name "White River City Investments" currently <br />has a status of "non- compliant" (Please see enclosed copy of the summary). The <br />application cannot be processed unless the applicant is in good standing with the <br />Colorado Secretary of State. <br />Your application will not be considered submitted until the information listed above is received <br />and found sufficient to begin our review. A decision date will be established 90 days from the <br />date of receipt of all of the requested information. Additionally, if you have already published <br />notice you will need to republish notice, after the Division considers the application submitted. <br />This notice must be published once a week for four (4) consecutive weeks, starting within ten <br />(10) days of the date your application is considered submitted.
